A great opportunity awaits for Nurse Practitioners passionate about providing patient-centered care in home settings. As a key healthcare provider, you will perform comprehensive home health risk assessments for Medicare Advantage members residing in Hamilton County, Indiana. Each assessment typically lasts about 45 minutes and may include annual health reviews or post-discharge visits. Your daily locations may vary among Carmel, Fishers, and Noblesville, with flexible scheduling that adapts to your availability.
Schedule:
- Flexible shifts offered Monday through Sunday
- Tailor your schedule to fit your needs
- Minimum weekly or bi-weekly hours required
Key Responsibilities:
- Conduct accurate and thorough home health risk assessments for Medicare Advantage beneficiaries
- Review medical history, ongoing medical conditions, and medication regimens
- Educate patients on their specific health conditions and medication management
- Spot care gaps and recommend solutions to improve patient outcomes
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- Current BLS certification
- Board certification through AANP or ANCC
- Preferred: At least 6 months of recent NP experience
- Master of Science in Nursing (MSN) from an accredited institution
Your expertise will be recognized with compensation of $80-$90 per evaluation, in addition to a mileage stipend and paid administrative time.
