Deliver essential care and insights directly to patients homes as an experienced Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members across Jasper County, MO. This per diem opportunity offers flexible scheduling with shifts available any day of the weekcreate your own schedule and cover visits in locations including Carthage, Webb City, Carterville, and the Joplin area. Enjoy autonomy and offer meaningful health guidance while providing an essential service.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule based on your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments in patients homes
- Review and evaluate prior/current medical conditions, personal and family medical history, and medication usage
- Educate patients about their medical conditions and medications
- Identify gaps in care and make actionable recommendations to improve patient outcomes
- Focus exclusively on assessmentno treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner license in Missouri
- Board Certification by AANP or ANCC
- Current CPR (BLS) certification
- Graduate of an accredited MSN (Master of Science in Nursing) program
- At least 6 months of recent NP experience preferred
This role offers compensation of $80$90 per evaluation, along with a mileage stipend and paid administrative time.
