Make an impact within your community by delivering high-quality care in a flexible per diem role as a Nurse Practitioner specializing in home health risk assessments. In this position, you will serve Medicare Advantage members across St. Landry County, including areas such as Opelousas, Eunice, and Port Barre. Each home health risk assessment typically takes about 45 minutes and may involve annual health evaluations or post-discharge visits. Based on scheduling needs, you could be assigned to various locations within the county for your shifts.
Schedule:
- Flexible shifts available Monday through Sunday
- Build your own schedule to fit your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
- Evaluate patients prior and current medical histories, medical conditions, and medication usage
- Deliver patient education regarding chronic and acute conditions, as well as prescribed medications
- Identify care gaps and recommend actionable solutions
- Focus exclusively on assessmentsno treatment planning or medication prescribing required
Preferred Qualifications:
- Unrestricted Nurse Practitioner license in Louisiana
- Board Certification through AANP or ANCC required
- Completion of an MSN degree from an accredited nursing school
- Current CPR (BLS) certification
- Minimum of six months recent NP experience is preferred
