Enhance patient outcomes and support community wellness as a Nurse Practitioner delivering home health risk assessments on a per diem basis. This opportunity allows you to leverage your clinical expertise by providing crucial assessments for Medicare Advantage plan members throughout Cape Girardeau County. Your visits will help ensure members receive the care and resources they need, whether through annual health checkups or post-discharge follow-ups.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Perform thorough home health risk assessments, averaging 45 minutes per visit
• Review and document medical, surgical, and medication histories
• Educate patients and caregivers about conditions, treatment options, and medications
• Identify gaps in healthcare delivery and provide actionable recommendations
• Manage administrative documentation related to each visit
• Note: This position does not involve prescribing medications or developing treatment plans

Qualifications:

• Active Nurse Practitioner license in Missouri
• Board Certification through AANP or ANCC
• Current BLS/CPR certification
• Minimum 6 months of recent experience as an NP preferred
• Master of Science in Nursing (MSN) from an accredited school
