This rewarding opportunity is tailored for a skilled Nurse Practitioner specializing in home health services. As an integral healthcare provider, you will deliver comprehensive home-based risk assessments for Medicare Advantage members across Cape Coral and greater Lee County. Enjoy a highly flexible per diem schedulework when it suits you, with the freedom to create your own shifts and select your commitment level weekly or bi-weekly.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own hours and manage your weekly workload
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct thorough annual home health risk assessments for Medicare Advantage participants
  • Review and document prior and current medical history, existing conditions, and medications
  • Educate patients about their diagnoses and medication regimens
  • Detect healthcare gaps and offer practical recommendations for enhanced patient outcomes
  • Visits generally last around 45 minutes; may include routine annual reviews or post-discharge assessments
  • No medication prescribing or treatment plan creation involvedfocus on evaluation and education only
  • Travel required throughout designated areasCape Coral, Fort Myers, and Lehigh Acres

Qualifications:

  • Valid Nurse Practitioner license in Florida
  • Current BLS/CPR certification
  • Board certified via AANP or ANCC
  • Six months or more of recent Nurse Practitioner experience preferred
  • MSN from an accredited nursing program

In addition to the flexibility and autonomy, this role offers compensation of $80-$90 per completed assessment, along with stipends for mileage and administrative duties.
