Healthcare professionals with experience in home health are invited to provide vital care as a Nurse Practitioner performing home health risk assessments. In this per diem role, you will visit Medicare Advantage plan members in various locations throughout Beckham County, including Elk City, Sayre, and Erick. Youll be instrumental in evaluating health status and identifying care needs, all while enjoying a flexible schedule that fits your lifestyle.
Schedule:
- Flexible shifts available seven days a week
- Build your own schedule
- Choose a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage members in their residences
- Review and document prior/current medical histories, ongoing conditions, and medication use
- Provide patient education regarding conditions and medication regimens
- Identify gaps in care and recommend appropriate next steps
- No responsibility for prescribing medications or developing treatment plans
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Current CPR certification (BLS required)
- Board Certification through AANP or ANCC
- At least 6 months of recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited institution
In addition to a mileage stipend and paid administrative time, compensation averages $80$90 per completed evaluation.
