Job Description
Seeking an experienced Nurse Practitioner to perform home health risk assessments on a per diem basis. This role supports Medicare Advantage members in the Summit County area, including cities like Akron, Cuyahoga Falls, Stow, and Barberton. Assessments typically last around 45 minutes and may include annual health reviews or post-discharge visits. Youll engage directly with patients in their homes to help improve health outcomes without the need to develop treatment plans or prescribe medications.
Schedule:
- Flexible shifts available seven days a week

- Choose a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members
- Review and evaluate patient medical history, current health status, and medications
- Educate patients on their health conditions and prescribed medications
- Identify gaps in care, offer recommendations, and empower patients with appropriate resources
- Documentation and completion of assessments; no prescribing or ongoing treatment required
Desired Qualifications:
- Active Nurse Practitioner license in Ohio
- Current BLS/CPR certification
- Board Certification via AANP or ANCC
- Minimum of six months NP experience preferred
- MSN from an accredited program
Compensation is offered at $80$90 per evaluation, along with a mileage stipend and compensation for administrative time.
