Bring your expertise in home health care to support Medicare Advantage plan members by performing comprehensive risk assessments in Stark County, Ohio. This rewarding per diem opportunity is ideal for Nurse Practitioners who value schedule flexibility and independent clinical work. Shifts are available across areas such as Canton, Massillon, North Canton, and Alliance, with the ability to build your own schedule and specify availability between Monday and Sunday. A minimum commitment is required on a weekly or bi-weekly basis, offering both convenience and consistency.
Schedule:

Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members in their homes
- Assess patients prior and current medical and medication histories
- Deliver education related to medical conditions and proper medication use
- Identify care gaps and supply actionable recommendations for members
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Valid Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months experience as an NP is preferred
