Enjoy the flexibility of per diem work as you provide meaningful care in a home health setting. Nurse Practitioners are needed to conduct in-home risk assessments for Medicare Advantage members across St. Lawrence County. Each visit typically takes around 45 minutes and may encompass annual health assessments or post-discharge evaluations. Assignments will be located throughout the region, with shifts available in communities like Canton, Massena, Potsdam, and Ogdensburg.
Schedule:
- Flexible shifts available any day from Monday through Sunday
- Self-select your workdays and hours
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Review medical history, current conditions, and medication usage
- Deliver education on conditions and prescribed medications
- Identify gaps in care and provide recommendations to improve patient outcomes
- No requirement to prescribe medications or create ongoing treatment plans
Qualifications:
- Active Nurse Practitioner license in New York
- Current CPR certification (BLS required)
- Board certification through AANP or ANCC
- At least 6 months of recent NP experience preferred
- MSN degree from an accredited nursing program
This per-evaluation, per diem position offers compensation of $80$90 per assessment, along with a provided mileage stipend and administrative time compensation.
