Job Description

Seeking an experienced Nurse Practitioner to perform home health risk assessments for a Medicare Advantage plan. This per diem opportunity allows you to make a meaningful impact by visiting members in their residences within Fremont County. Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own hours to fit your lifestyle
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Visit Medicare Advantage plan members to complete annual or post-discharge health risk assessments
  • Carefully review prior and current medical history, medical conditions, and medication use
  • Offer education to patients regarding their conditions and medications
  • Identify gaps in care and recommend next steps for healthcare follow-up
  • No responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Colorado
  • Valid CPR certification (BLS)
  • Board certified via AANP or ANCC
  • Minimum of 6 months nurse practitioner experience preferred
  • MSN degree from an accredited nursing program
