Seeking an experienced Nurse Practitioner to perform home health risk assessments for a Medicare Advantage plan. This per diem opportunity allows you to make a meaningful impact by visiting members in their residences within Fremont County. Schedule:
- Flexible shifts available Monday through Sunday
- Set your own hours to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Visit Medicare Advantage plan members to complete annual or post-discharge health risk assessments
- Carefully review prior and current medical history, medical conditions, and medication use
- Offer education to patients regarding their conditions and medications
- Identify gaps in care and recommend next steps for healthcare follow-up
- No responsibility for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Colorado
- Valid CPR certification (BLS)
- Board certified via AANP or ANCC
- Minimum of 6 months nurse practitioner experience preferred
- MSN degree from an accredited nursing program
