Job Description

Make a meaningful impact as a Nurse Practitioner supporting Medicare Advantage members through in-home health risk assessments in Mahoning County and nearby areas. This per diem position offers true flexibility and the chance to use your clinical expertise in a patient-focused, quality-driven setting.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule
  • Choice of minimum weekly or bi-weekly hourly commitments

In this role, you will:

  • Perform home health risk assessments for Medicare Advantage members in a variety of settings, including annual health checks and post-discharge visits.
  • Review and assess prior and current medical history, conditions, and medication usage.
  • Provide patient education regarding their diagnoses and medications.
  • Identify care gaps and recommend follow-up to ensure optimal outcomes.
  • No treatment plan creation or medication prescribing is required.

Desired qualifications:

  • Nurse Practitioner License valid in the state of Ohio
  • Current CPR certification (BLS)
  • Board Certified by AANP or ANCC
  • At least six months experience as a Nurse Practitioner preferred
  • MSN from an accredited nursing program

This highly autonomous position provides compensation of $80$90 per evaluation completed, along with a mileage stipend and administrative time, acknowledging the value of your dedication and expertise.
