Enjoy a rewarding per diem opportunity as an experienced Nurse Practitioner specializing in home health risk assessments. In this flexible position, youll conduct comprehensive annual or post-discharge health assessments for Medicare Advantage plan members throughout Columbiana County. Locations may include Perry Township, Salem, and East Liverpool, with each visit averaging 45 minutes. Your clinical expertise will help identify care gaps and enhance the well-being of individuals in their homes.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimum commitment required on a weekly or bi-weekly basis
Key Responsibilities:
- Conduct thorough home health risk assessments for Medicare Advantage members
- Review medical, medication, and treatment histories
- Provide patient education regarding conditions and medication regimens
- Identify and address potential care gaps, offering recommendations as needed
- Documentation and reporting for each assessment
- No requirement to prescribe medications or develop treatment plans
Qualifications:
- Valid Nurse Practitioner License in Ohio
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Six months of recent NP experience preferred
