Take on a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage members. This vital role involves visiting patients in their residences throughout Henderson County, where you'll complete thorough, patient-focused evaluations that help ensure quality care and support wellness initiatives.
Schedule/Hours:
- Flexible shifts offered Monday through Sunday
- Ability to create your own schedule
- Only a minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete comprehensive home health risk assessments, generally within 45 minutes, for individuals enrolled in a Medicare Advantage plan
- Review and evaluate prior and current medical history, ongoing medical conditions, and detailed medication lists
- Deliver clear education to patients regarding their health conditions and medications
- Identify care gaps and provide actionable recommendations, collaborating to help patients navigate their health journey
- Visits may cover various scenarios, including annual health assessments or post-discharge visits
- No prescribing medications or creating treatment plans is involved
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- MSN degree from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
