Job Description
Take on an impactful role providing home health risk assessments for Medicare Advantage plan members throughout Pittsburg County, OK, including locations such as McAlester, Hartshone, Krebs, and Quinton. As a Nurse Practitioner specializing in Home Health, you will play a key role in evaluating and educating patients within their own homes, ensuring quality care and improved health outcomes.
Schedule:
- Flexible shifts available seven days a week
- Set your own schedule
- Choose from minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform comprehensive annual home health risk assessments
- Evaluate prior and current medical history, existing medical conditions, and medication use
- Educate patients about their medical conditions and appropriate medications
- Identify care gaps and provide recommendations to improve patient outcomes
- Documentation of assessments; no requirement for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Current CPR certification (BLS required)
- Board Certification through AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience (preferred)
- Master of Science in Nursing (MSN) from an accredited institution
This position offers compensation of $80-$90 per evaluation, in addition to a mileage stipend and administrative time for each completed assessment.
