Job Description

Schedule:

  • Flexible shifts available any day between Monday and Sunday
  • Design your own work schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct thorough annual risk assessments for Medicare Advantage members in a home setting
  • Review and evaluate patients medical history, current and prior conditions, and medication profiles
  • Educate patients about their health conditions and medications, fostering patient engagement
  • Identify care gaps and recommend next steps to address potential health risks
  • Visits may include annual wellness assessments or post-discharge evaluations; each typically lasts about 45 minutes
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Unencumbered Nurse Practitioner license in Virginia
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited institution
  • At least 6 months of NP experience is preferred

You will receive compensation of $80$90 per evaluation, along with a mileage stipend and payment for administrative time.
