Take on an important role providing home health risk assessments as a Nurse Practitioner in the Carroll County area. Schedule:
- Flexible shifts available any day between Monday and Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct thorough annual risk assessments for Medicare Advantage members in a home setting
- Review and evaluate patients medical history, current and prior conditions, and medication profiles
- Educate patients about their health conditions and medications, fostering patient engagement
- Identify care gaps and recommend next steps to address potential health risks
- Visits may include annual wellness assessments or post-discharge evaluations; each typically lasts about 45 minutes
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Unencumbered Nurse Practitioner license in Virginia
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited institution
- At least 6 months of NP experience is preferred
You will receive compensation of $80$90 per evaluation, along with a mileage stipend and payment for administrative time.
