Schedule/Hours:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments in members homes, each typically taking about 45 minutes
  • Review and document prior/current medical history, conditions, and medications
  • Provide education regarding health conditions and prescribed treatments
  • Pinpoint gaps in care, making informed recommendations to enhance outcomes
  • Note: No development of treatment plans or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Board Certification from AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited nursing program
  • At least 6 months of recent NP experience preferred
