Schedule/Hours:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments in members homes, each typically taking about 45 minutes
- Review and document prior/current medical history, conditions, and medications
- Provide education regarding health conditions and prescribed treatments
- Pinpoint gaps in care, making informed recommendations to enhance outcomes
- Note: No development of treatment plans or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Illinois
- Board Certification from AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least 6 months of recent NP experience preferred
