Deliver comprehensive patient evaluations in a flexible, rewarding per diem opportunity as an experienced Nurse Practitioner specializing in home health risk assessments. This role centers on in-home visits to Medicare Advantage plan members across Adams County, including Quincy, Camp Point, and Payson. With the freedom to create your own schedulechoosing from shifts available any day between Monday and Sundaythis position fits a variety of lifestyles and offers minimum hourly commitments on a weekly or bi-weekly basis.
Key Responsibilities:
- Conduct in-depth annual and post-discharge home health risk assessments, averaging 45 minutes per visit
- Review patients prior and current medical and medication history
- Educate patients on their health conditions and prescribed medications
- Identify gaps in care and recommend appropriate measures for care optimization
- Deliver assessments without the expectation to prescribe medications or create detailed treatment plans
Qualifications:
- Active and unencumbered Nurse Practitioner license in Illinois
- Current CPR/BLS certification
- Board certification via AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
- Masters degree in Nursing (MSN) from an accredited institution
