Job Description

Advance your nursing career in a dynamic per diem role, conducting comprehensive home health risk assessments for Medicare Advantage plan members throughout Onslow County, NC. Leverage your expertise as a Nurse Practitioner by providing meaningful care directly in patient homes, helping to ensure quality, continuity, and safety for a diverse population.

Flexible Schedule:

  • Choose your own shifts, available Monday through Sunday
  • Set your schedule to accommodate weekly or bi-weekly minimum hourly commitments

Primary Responsibilities:

  • Perform annual and post-discharge home health risk assessments, averaging 45 minutes each
  • Review and evaluate medical history, conditions, and medications for each patient
  • Deliver clear education and guidance regarding health conditions and prescribed medications
  • Identify any gaps in care and provide actionable recommendations to optimize patient wellness
  • Note: This position does not require the creation of treatment plans or the prescribing of medications

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Minimum six months of recent nurse practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited school
