This opportunity allows you to serve Medicare Advantage members in various communities, conducting visits in residential settings throughout Cayuga County, including Auburn, Cayuga, Union Springs, and Moravia. Enjoy the flexibility to shape your own schedule with shifts available between Monday and Sunday, and the ability to commit weekly or bi-weekly hours based on your availability.
Schedule:
- Flexible shifts offered Monday through Sunday
- Choose your preferred working days
- Minimum hourly commitments determined on a weekly or bi-weekly basis
Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage members
- Review and assess current and previous medical history, conditions, and medication use
- Provide education regarding health conditions, medications, and preventive care
- Identify care gaps and recommend appropriate next stepsno prescribing or treatment planning required
- Complete documentation accurately after each 45-minute evaluation
Qualifications:
- Active Nurse Practitioner license in New York
- MSN from an accredited nursing program
- Board certified by AANP or ANCC
- Current CPR (BLS) certification
- At least 6 months of Nurse Practitioner experience preferred
