Job Description
Make a meaningful impact in community health by conducting home visits as an experienced Nurse Practitioner. This rewarding per diem opportunity involves performing home health risk assessments for Medicare Advantage plan members throughout Kent County.
Schedule:

Responsibilities include:
- Perform annual health risk assessments in patient homes
- Review and document prior and current medical history and conditions
- Discuss and educate patients on their diagnoses, medications, and preventive health
- Identify care gaps and make recommendations for improved health outcomes
- No requirement for treatment planning or medication prescribing
Desired Qualifications:
- Active, unencumbered Nurse Practitioner license in Delaware
- Board certification through AANP or ANCC
- Completed MSN from an accredited nursing program
- Current CPR (BLS) certification
- Minimum of six months experience as an NP preferred, but not required
As part of this role, you will visit patients located throughout Kent County and its communities, including Dover, Smyrna, and Milford. Each risk assessment usually takes about 45 minutes, covering routine annual evaluations and post-discharge check-ins. Compensation is highly competitive, at $80-$90 per completed assessment, plus a mileage stipend and payment for administrative duties.
