Enhance the well-being of individuals in Kershaw County by providing vital home health assessments as a Nurse Practitioner. This per diem position allows you to take control of your schedule, working flexible shifts between Monday and Sunday, with a minimum weekly or bi-weekly hourly commitment tailored to your availability.

What You'll Do:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members in Camden, Lugoff, Bethune, and surrounding Kershaw County communities
- Review medical history, current conditions, and medication usage during each 45-minute in-home evaluation
- Provide essential education regarding chronic conditions, medication adherence, and overall health management
- Identify gaps in care and offer tailored recommendations to improve patient outcomes
- No requirement to create treatment plans or prescribe medications
Desired Qualifications:
- Active Nurse Practitioner License in South Carolina
- Board Certified via AANP or ANCC
- Current CPR (BLS) certification
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of current Nurse Practitioner experience preferred
