Job Description

Take on a dynamic, patient-facing role providing home health risk assessments to Medicare Advantage plan members in Ouachita County, AR. Utilizing your skills as a Nurse Practitioner, youll help enhance care quality and patient outcomes through focused health evaluations in the community. This per diem position offers maximum flexibilityset your own schedule and make a difference for patients across locations such as Camden, Stephens, and Bearden.

Schedule

  • Flexible shifts available Monday through Sunday
  • Self-scheduling: design your own work calendar
  • Meet a minimum weekly or bi-weekly hourly commitment

Key Responsibilities

  • Conduct annual and post-discharge risk assessments for Medicare Advantage members at their residences
  • Review and document current and prior medical and medication histories
  • Deliver patient education regarding chronic conditions and medications
  • Identify gaps in care and suggest appropriate recommendations (no treatment plan creation or prescribing involved)
  • Complete each assessment in approximately 45 minutes

Qualifications

  • Active Nurse Practitioner license in Arkansas (AR)
  • Board Certification through AANP or ANCC
  • Current BLS/CPR certification
  • MSN obtained from an accredited nursing program
  • At least 6 months of current nurse practitioner experience preferred
