Make a meaningful difference in patient lives by leveraging your expertise as a Nurse Practitioner in a flexible, per diem home health setting. In this role, you will conduct comprehensive home health risk assessments for Medicare Advantage members residing throughout Guernsey County, including Cambridge Township, Cambridge, and Jackson Township. Assessments typically last around 45 minutes and vary in nature, such as annual health reviews or post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday

- Set your own minimum weekly or bi-weekly commitment
Core Responsibilities:
- Complete annual risk assessments for Medicare Advantage plan members in their homes
- Evaluate patients medical and medication histories
- Educate members about their health conditions and medications
- Identify care gaps and make recommendations to improve patient outcomes
- No requirement to create treatment plans or prescribe medications
Required Qualifications:
- Active Nurse Practitioner license (OH)
- Current BLS/CPR certification
- Board certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Minimum of 6 months of current Nurse Practitioner experience preferred
