Make a meaningful impact by providing in-home risk assessments for Medicare Advantage plan members as a Nurse Practitioner. In this per diem position, youll play a vital role by ensuring patients receive high-quality, individualized care within their homes across Boston, Revere, Chelsea, and other areas in Suffolk County, Massachusetts.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-directed schedulingset your own work hours
• Maintain a minimum weekly or bi-weekly hour commitment

Primary Responsibilities:

• Complete comprehensive annual health and post-discharge risk assessments in patient homes
• Review and evaluate medical history, current and prior medical conditions, and medication usage
• Educate patients on their health conditions, treatments, and medication regimens
• Identify care gaps and suggest appropriate follow-up actions
• No treatment planning or prescribing responsibilities

Qualifications:

• Current Nurse Practitioner license in Massachusetts
• Active CPR certification (BLS)
• Board certified through AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited institution
• Minimum six months of recent NP experience strongly preferred
