Job Description

Step into an impactful per diem opportunity conducting in-home health risk assessments for Medicare Advantage plan members across Middlesex County, Massachusetts. As a Nurse Practitioner, you will use your clinical expertise to deliver quality care through thorough assessments, patient education, and recommendations, all in the comfort of patients homes in areas such as Cambridge, Lowell, and Newton.

Schedule:

  • Flexible shifts available any day of the week, Monday through Sunday
  • Set your own hours and manage your workload
  • Minimum commitment on a weekly or bi-weekly basis

Key Responsibilities:

  • Conduct comprehensive annual health risk assessments in patient homes
  • Review medical history, current conditions, and medication use
  • Educate patients regarding their health conditions and medications
  • Identify care gaps and suggest actionable steps to address them
  • Focus is solely on assessmentno prescription writing or development of treatment plans required

Qualifications:

  • Active Nurse Practitioner license in Massachusetts
  • CPR certification (BLS) required
  • Certified by AANP or ANCC
  • Minimum of 6 months of recent NP experience preferred
  • MSN degree from an accredited nursing program

This role offers a pay structure of $80$90 per completed evaluation, in addition to compensation for mileage and administrative time.
