Advance your nurse practitioner career through impactful home health visits, focusing on risk assessments for Medicare Advantage plan members in Henry County. Schedule:

Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Assess patients' current and prior medical history, health conditions, and medication usage
- Deliver education regarding medical conditions and medications to improve outcomes
- Identify care gaps and provide actionable recommendations for resolution
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- Current BLS/CPR certification
- Board certification from AANP or ANCC
- Prefer at least 6 months of recent experience as a Nurse Practitioner
- MSN from an accredited nursing school
Compensation for each evaluation ranges from $80 to $90, with an included mileage stipend and consideration for administrative time.
