Job Description

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage plan members
  • Assess patients' current and prior medical history, health conditions, and medication usage
  • Deliver education regarding medical conditions and medications to improve outcomes
  • Identify care gaps and provide actionable recommendations for resolution
  • No requirement for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current BLS/CPR certification
  • Board certification from AANP or ANCC
  • Prefer at least 6 months of recent experience as a Nurse Practitioner
  • MSN from an accredited nursing school

Compensation for each evaluation ranges from $80 to $90, with an included mileage stipend and consideration for administrative time.
