Job Description
Deliver compassionate, high-quality care while enjoying outstanding flexibility as a Nurse Practitioner specializing in home health risk assessments on a per diem basis. In this role, you will leverage your clinical expertise to conduct comprehensive, in-home risk evaluations for Medicare Advantage members across the Robertson County, TX areaincluding Franklin, Calvert, Hearne, and Bremond. Each patient visit typically lasts around 45 minutes and may be focused on annual assessments or post-discharge follow-ups.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling with your choice of hours
- Minimum weekly or bi-weekly commitment required
Key Responsibilities:
- Perform detailed annual risk assessments on health plan members in their homes
- Review medical and medication history, including prior and current conditions
- Educate patients on their diagnoses, treatment options, and prescriptions
- Identify care gaps and make recommendations for appropriate follow-up
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Unrestricted Nurse Practitioner License in Texas
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- Masters Degree in Nursing (MSN) from an accredited institution
- At least 6 months of recent experience as a Nurse Practitioner preferred
