Job Description
Seeking an experienced Nurse Practitioner to perform home health risk assessments for Medicare Advantage members in Daviess County, Kentucky. This per diem opportunity offers flexibility and the chance to provide essential care to patients in cities such as Owensboro, Whitesville, and Maceo. Assessments typically last around 45 minutes and may include either annual health reviews or post-discharge visits in patient homes.

Key Responsibilities:
- Complete annual risk assessments for Medicare Advantage health plans
- Evaluate prior and current medical history, chronic conditions, and medication use
- Deliver clear education on health conditions and medication management
- Identify gaps in care and provide practical recommendations to improve health outcomes
- Note: No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License in Kentucky
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- At least six months of recent NP experience preferred
Compensation includes $80$90 per evaluation, along with a mileage stipend and payment for administrative time associated with each visit.
