Schedule:

• Flexible shifts available between Monday and Sunday
• Design your own work schedule
• Minimum weekly or bi-weekly hourly commitments

Primary Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage plan members in locations across El Paso County, including Colorado Springs, Fountain, and Monument
• Assess prior and current medical history, health conditions, and medication usage
• Educate members on health conditions, medication adherence, and preventative care
• Identify gaps in care, recommending practical solutions to enhance patient outcomes
• Visits are primarily assessmentsno need to create treatment plans or prescribe medication

Qualifications:

• Active Nurse Practitioner license in Colorado
• Board certification via AANP or ANCC
• Current BLS/CPR certification
• MSN from an accredited nursing program
• At least 6 months of recent NP experience (preferred)
