Step into a rewarding per diem opportunity where your expertise as a Nurse Practitioner makes a meaningful difference. This role involves conducting in-home health risk assessments for Medicare Advantage plan members throughout Macon County, including locations such as La Plata, Bevier, Atlanta, and surrounding areas. Each evaluation typically lasts about 45 minutes and may encompass annual health assessments or post-discharge visits.

Schedule:

• Flexible coverage between Monday and Sunday
• Self-scheduling allows you to create your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members
• Assess and document patients prior and current medical history, health conditions, and medication use
• Educate patients on their conditions and medication regimens to enhance understanding and adherence
• Identify care gaps and recommend interventions to close these gaps
• Note: This position does not require treatment planning or medication prescribing

Qualifications:

• Active Nurse Practitioner license in Missouri
• Current BLS/CPR certification
• Board certification from AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited institution
• At least 6 months of current NP experience (preferred)

This per diem position offers competitive compensation of $80$90 per completed evaluation, complemented by a mileage stipend and administrative time allowance.
