Seeking a qualified Nurse Practitioner to conduct home health risk assessments within Fentress County, TN. This per diem position involves visiting Medicare Advantage plan members in their homes for comprehensive health evaluations. Each assessment typically lasts about 45 minutes and may include annual health check-ups or post-discharge follow-ups. Scheduling is flexible, allowing you to create shifts that fit your availability. You may be assigned to locations such as Jamestown, Allardt, or Grimsley during any given shift.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual risk assessments for a Medicare Advantage health plan
- Review patient medical history, current conditions, and medication use
- Offer education regarding diagnoses and medications
- Detect gaps in care and suggest actionable recommendations
- No requirement to prescribe medications or create treatment plans
Qualifications:
- Current Nurse Practitioner License in Tennessee
- Active CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of NP experience preferred
