Job Description
As a Nurse Practitioner specializing in home health, you will play a key role in conducting comprehensive home health risk assessments for Medicare Advantage plan members across Genesee County, including locations such as Flint, Grand Blanc, and Burton. This per diem opportunity offers the flexibility to create your own schedule while making meaningful contributions to patient care in the community.
Schedule:
- Flexible shifts available between Monday and Sunday
- Design your own schedule to fit your availability
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual health risk assessments in members homes, each typically lasting about 45 minutes
- Review and evaluate both current and past medical histories, medical conditions, and medication regimens
- Provide patient education on health conditions and medications to ensure understanding and compliance
- Identify gaps in patient care and recommend strategies for resolution
- Conduct assessments onlyno requirement for treatment plans or prescription duties
Qualifications:
- Active Nurse Practitioner license in Michigan
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- Master of Science in Nursing (MSN) degree from an accredited institution
- At least 6 months of recent Nurse Practitioner experience preferred
