Deliver impactful healthcare to your community as a dedicated Nurse Practitioner conducting home health risk assessments throughout Genesee County, MI. This vital, per diem opportunity allows you to partner with Medicare Advantage plan members to support comprehensive, preventive care from the comfort of their own homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling: design your own work schedule
- Choose weekly or bi-weekly minimum hourly commitments based on preferences
Core Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members across Flint, Grand Blanc, Burton, and surrounding areas
- Assess and evaluate prior and current medical history, chronic conditions, and medication regimens during each 45-minute visit
- Educate patients about their health conditions, care plans, and medication adherence
- Identify and address any care gaps, providing clear recommendations for optimal health management
- Focus solely on assessmentsno need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Michigan
- Board Certification through AANP or ANCC required
- Master of Science in Nursing (MSN) from an accredited school
- Current BLS/CPR certification
- At least 6 months of recent Nurse Practitioner experience preferred
