This per diem opportunity offers nurse practitioners the chance to provide vital home health risk assessments throughout Genesee County, encompassing areas including Flint, Grand Blanc, and Burton.

Schedule:

• Flexible shifts available throughout the week (Monday to Sunday)
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete home health risk assessments for Medicare Advantage plan members in various communities within Genesee County.
• Conduct reviews of patients prior and current medical histories, conditions, and medication usage.
• Deliver clear education to patients regarding their medical conditions and prescribed medications.
• Identify any care gaps and offer informed recommendations to address them.
• Visits may include annual assessments or post-discharge evaluations.
• No requirement to create treatment plans or prescribe medications.

Qualifications:

• Active Nurse Practitioner license in Michigan
• Board Certification from AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing program
• Six months or more of recent NP experience preferred
