Job Description

This per diem opportunity is ideal for an experienced Nurse Practitioner seeking a flexible, independent position focusing on home health risk assessments within Yancey County. Providers will conduct in-home evaluations for Medicare Advantage plan members in various communities, including Cane River, Bald Creek, Celo, Sioux, and the broader Yancey County areas. Each visit lasts approximately 45 minutes and may involve annual health assessments or post-discharge evaluations, helping to ensure the well-being and continuity of care for patients in their homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling for maximum autonomy
  • Weekly or bi-weekly minimum hourly commitments

Key Responsibilities:

  • Complete annual health risk assessments for Medicare Advantage members in their homes
  • Review and evaluate patient medical histories, current conditions, and medication usage
  • Provide patient education concerning health conditions and prescribed medications
  • Identify gaps in care and recommend appropriate follow-up actions
  • No treatment plan development or medication prescribing is required

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current BLS/CPR certification
  • Board Certification through AANP or ANCC
  • Minimum of six months recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution

You will be compensated $80$90 for each completed evaluation, with additional stipends provided for mileage and administrative time. This role grants significant flexibility, letting you design your ideal work schedule while making a meaningful impact on the lives of Medicare Advantage members.
