Step into a rewarding per diem opportunity as a Nurse Practitioner delivering vital home health risk assessments to Medicare Advantage plan members throughout Madison County, NC. This position offers you full flexibility: create your own schedule, with shifts available any day of the week, Monday through Sunday. Youll have the autonomy to set a minimum weekly or bi-weekly hourly commitment, and each assessment will typically take about 45 minutes. Locations covered include Mars Hill, Marshall, Hot Springs, and neighboring communities within Madison County.
Key Responsibilities
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
- Review medical histories, current health status, and medication use
- Teach patients about medical conditions and medications during visits
- Identify care gaps and make evidence-based recommendations for follow-up
- No requirement for treatment plans or prescribingfocus is on assessment and education

Required Qualifications
- Valid Nurse Practitioner License in North Carolina
- Active CPR (BLS) certification
- Board Certification (AANP or ANCC)
- MSN degree from an accredited nursing program
- At least 6 months of current NP experience preferred
