Make a meaningful impact in patient care by performing home health risk assessments for Medicare Advantage plan members throughout Madison County, NC. This per diem opportunity allows you to use your clinical skills in a flexible setting with the autonomy to create your own schedule. Serve diverse communities, including Mars Hill, Marshall, and Hot Springs, conducting visits that last approximately 45 minutes each.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule to maintain work-life balance
- Choose your minimum weekly or bi-weekly hours
Key Responsibilities:
- Complete comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate prior medical history, chronic conditions, and medication usage
- Educate patients on health conditions and medication management
- Identify care gaps and make recommendations to improve patient outcomes
- Focus on assessment and health education onlyno prescription or treatment planning involved
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current BLS/CPR certification
- Board Certification by AANP or ANCC
- MSN from an accredited nursing school
- At least 6 months of recent nurse practitioner experience (preferred)
