Job Description

Deliver compassionate care in a dynamic, flexible environment as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity offers autonomy and the ability to make a meaningful impact on Medicare Advantage plan members across Beckham County, including areas such as Elk City, Sayre, and Erick. You will play a vital role in enhancing patient outcomes through comprehensive home visits while enjoying a self-created schedule that fits your lifestyle.

Schedule:

  • Flexible shifts available any day, Monday through Sunday
  • Design your own schedule
  • Commit to weekly or bi-weekly minimum hours

Key Responsibilities:

  • Perform annual risk assessments for Medicare Advantage members in their homes
  • Review and evaluate patients medical history, conditions, and medications
  • Deliver individualized patient education regarding health conditions and medication management
  • Identify gaps in patient care and recommend solutions to improve outcomes
  • Visits focus on assessmentno prescribing medications or developing treatment plans

Qualifications:

  • Active Nurse Practitioner license in Oklahoma
  • MSN degree from an accredited nursing program
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • At least six months of recent NP experience preferred
