Make a meaningful impact in patients lives by providing high-quality home health assessments as a Nurse Practitioner specializing in home health visits. This per diem opportunity empowers you with scheduling flexibility while serving Medicare Advantage plan members in Minidoka County, including Rupert, Heyburn, and Paul.
Schedule:
- Flexible shifts available Monday through Sunday
- Craft your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments in patients homes
- Review and assess prior and current medical history, conditions, and medications
- Deliver personalized education on health conditions and prescription management
- Identify and report any gaps in care, offering recommendations for follow-up
- No prescription writing or detailed treatment planning required
Qualifications:
- Active Nurse Practitioner license in Idaho
- CPR certification (BLS) required
- Board Certification via AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
This position offers a robust compensation structure, providing $80$90 per evaluation, complemented by a mileage stipend and administrative time compensation. Each assessment typically requires approximately 45 minutes. Youll have the unique autonomy to choose your own shifts, providing crucial support to patients during annual or post-discharge visits in their homes.
