Job Description

Deliver high-quality patient care through home health risk assessments across Cassia County, serving Medicare Advantage plan members in their homes. As a Nurse Practitioner specializing in home health, youll conduct evaluations that typically take about 45 minutes, covering annual health assessments or post-discharge visits. You will engage with patients in Burley, Oakley, Declo, and the broader Cassia County area, with flexibility to be scheduled in different locations during each shift.

Responsibilities:

  • Complete comprehensive annual home health risk assessments for a Medicare Advantage health plan
  • Assess previous and current medical history, health conditions, and medication usage
  • Educate patients on health conditions, preventive care, and proper use of medications
  • Detect care gaps and offer actionable recommendations for improved outcomes
  • Prescribing medications and developing treatment plans are not part of this position

Qualifications:

  • Valid Nurse Practitioner license in Idaho
  • Active CPR certification (BLS)
  • Board Certification via AANP or ANCC
  • At least 6 months of recent experience as a Nurse Practitioner is preferred
  • Master of Science in Nursing (MSN) from an accredited institution
