Job Description
Deliver impactful patient care in a flexible, autonomous environment as a Nurse Practitioner specializing in home health risk assessments. In this per diem position, you will travel within Duplin County, visiting Medicare Advantage plan members in their homes. Each visit involves performing a thorough risk assessment, taking approximately 45 minutes, which may include annual health reviews or post-discharge evaluations. Locations served include Wallace, Mount Olive, and Warsaw, with the opportunity to create your own schedule for optimal work-life balance.
Schedule:
- Flexible shifts available any day of the week, Monday through Sunday
- Ability to make your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct comprehensive in-home health risk assessments for Medicare Advantage patients
- Evaluate patient medical history, assess current medical conditions, and review medication usage
- Educate patients on health conditions and appropriate medication management
- Identify gaps in care and offer recommendations to address these needs
- Focus entirely on assessmentsno requirement to prescribe medications or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in the state of North Carolina
- Current CPR certification (BLS)
- Board Certification through either AANP or ANCC
- At least 6 months of recent nurse practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
Compensation for this role includes $80$90 per completed evaluation, along with a mileage stipend and pay for administrative time.
