Job Description

Advance your clinical expertise by performing in-depth home health risk assessments for Medicare Advantage members across Duplin County, NC. This per diem opportunity focuses on providing high-quality, patient-centered evaluations to support ongoing care, without the demands of ongoing treatment planning or prescribing.

Schedule/Hours:

  • Flexible shifts available Monday through Sunday
  • Self-directed schedulingdesign shifts to suit your availability
  • Minimum weekly or bi-weekly hourly commitment to fit your lifestyle

Key Responsibilities:

  • Complete annual health assessments and post-discharge visits in patient homes throughout areas including Mount Olive, Wallace, and Warsaw
  • Assess medical history, current conditions, and medication use in a compassionate, thorough manner (average evaluation ~45 minutes)
  • Offer guidance and patient education on health conditions and prescribed medications
  • Detect care gaps and supply actionable recommendations to improve patient outcomes
  • Focus exclusively on assessmentno treatment planning or prescribing involved

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board certification through AANP or ANCC
  • Graduate of an accredited MSN program
  • Six months of recent nurse practitioner experience preferred
  • Current BLS (CPR) certification
