Embark on a rewarding opportunity as a Nurse Practitioner specializing in home health within Flagler County. In this per diem role, youll conduct thorough risk assessments for Medicare Advantage members in locations including Palm Coast, Flagler Beach, and Bunnell. The nature of each visit may range from annual health assessments to post-discharge check-ins, typically lasting around 45 minutes. Youll play a key part in promoting preventative care, helping patients understand their health, and guiding them toward improved well-beingall without the added responsibilities of treatment planning or prescribing.

- Choose between weekly or bi-weekly minimum commitment
Primary Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Review and evaluate previous and current medical history, existing conditions, and medication usage
- Educate patients on their medical conditions and prescribed medications
- Pinpoint potential gaps in care and efficiently recommend next steps
- Administrative documentation included; no ongoing treatment or prescribing obligations
Qualifications:
- Active Nurse Practitioner License in Florida
- Current Basic Life Support (BLS/CPR) certification
- Board Certification through AANP or ANCC
- Masters degree in nursing (MSN) from an accredited institution
- Minimum of 6 months experience practicing as an NP (preferred)
