Schedule:

• Flexible shifts available any day, Monday through Sunday
• Self-scheduling optionschoose the hours and days that fit you best
• Minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Perform annual home health risk assessments with Medicare Advantage plan members in locations throughout Flagler County, including Palm Coast, Flagler Beach, and Bunnell
• Conduct thorough evaluations of medical history, current conditions, and medication use
• Educate patients about their health conditions and prescribed medications
• Identify gaps in care and offer recommendations for improvement
• Visits may include annual assessments or post-discharge evaluations; each typically lasts about 45 minutes
• Note: This position does not require you to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Florida
• Current certification in Basic Life Support (CPR/BLS)
• Board certification via AANP or ANCC
• Prefer at least 6 months of recent experience as an NP
• MSN degree from an accredited nursing program

For each completed evaluation, youll receive competitive compensation in the $80$90 range, with a mileage stipend and administrative time included.
