Make a meaningful impact by providing in-home health assessments focused on the well-being of Medicare Advantage plan members throughout Cass County. As a Nurse Practitioner, youll conduct comprehensive risk assessments in patients' homesprimarily annual evaluations or post-discharge visitshelping bridge gaps in care and support quality outcomes. The per diem structure offers the flexibility to tailor your schedule while expanding your healthcare experience.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedule shifts to fit your life
- Meet minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform in-depth annual home health risk assessments for Medicare Advantage members in their residences
- Review patients medical history, current health status, and medication usage
- Educate individuals on medical conditions and proper medication handling
- Assess and highlight any gaps in care, offering actionable recommendations for improvement
- Note: There is no requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- Certification in Basic Life Support (BLS/CPR)
- National board certification via AANP or ANCC
- At least 6 months of current experience as a Nurse Practitioner (preferred)
- Master of Science in Nursing (MSN) from an accredited program
As a valued per diem clinician, youll receive $80$90 per completed evaluation in addition to a mileage stipend and compensation for administrative time.
