A per diem opportunity is available for an independent and motivated Nurse Practitioner with experience in home health settings. In this role, you will conduct risk assessments for Medicare Advantage plan members within Cass County, IN, covering communities such as Logansport, Galveston, and Walton. Assessments may include annual health evaluations or post-discharge follow-ups, with each visit lasting approximately 45 minutes.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitments required
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage members
- Review and assess prior and current medical histories, ongoing medical conditions, and medication usage
- Educate patients on health conditions and proper medication management
- Identify gaps in healthcare and make recommendations for follow-up or additional care
- No responsibility for treatment planning or prescription of medication
Qualifications:
- Active Nurse Practitioner license in Indiana
- Valid CPR certification (BLS)
- Board Certification through AANP or ANCC
- Completion of a MSN degree from an accredited nursing school
- At least 6 months of recent experience as a Nurse Practitioner preferred
Compensation and Perks:
Provide risk assessments at a rate of $80$90 per evaluation, complemented by a mileage stipend and administrative compensation for your time.
