Job Description
Deliver quality care and support to Medicare Advantage plan members as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity centers on providing focused, patient-centered assessments within Twin Falls County, including communities such as Twin Falls, Jerome, and Filer. You will have the flexibility to design your own schedule, working a minimum number of hours weekly or bi-weekly, with shifts available any day between Monday and Sunday.

Key Responsibilities:
- Conduct home health risk assessments for Medicare Advantage health plan members during annual or post-discharge visits
- Review each patients medical history, existing medical conditions, and medication profile
- Educate patients on their medical conditions and prescribed medications
- Identify gaps in care and offer actionable recommendations to support patient health outcomes
- Assessment visits average approximately 45 minutes
- No prescribing medications or creating treatment plans required
Qualifications:
- Active Nurse Practitioner license in Idaho
- Current CPR certification (BLS)
- Board Certified through AANP or ANCC
- Minimum of 6 months recent NP experience preferred
- MSN degree from an accredited institution
