Job Description

Deliver quality care and support to Medicare Advantage plan members as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity centers on providing focused, patient-centered assessments within Twin Falls County, including communities such as Twin Falls, Jerome, and Filer. You will have the flexibility to design your own schedule, working a minimum number of hours weekly or bi-weekly, with shifts available any day between Monday and Sunday.

Schedule:

  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct home health risk assessments for Medicare Advantage health plan members during annual or post-discharge visits
  • Review each patients medical history, existing medical conditions, and medication profile
  • Educate patients on their medical conditions and prescribed medications
  • Identify gaps in care and offer actionable recommendations to support patient health outcomes
  • Assessment visits average approximately 45 minutes
  • No prescribing medications or creating treatment plans required

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Current CPR certification (BLS)
  • Board Certified through AANP or ANCC
  • Minimum of 6 months recent NP experience preferred
  • MSN degree from an accredited institution
