This per diem opportunity empowers you to shape your own schedule, providing the flexibility to work shifts between Monday and Sunday and to tailor your hours on a weekly or bi-weekly basis.
Key Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage members
- Review and assess prior/current medical history, medical conditions, and medication usage
- Educate patients on medical conditions and prescribed medications
- Detect care gaps and offer actionable recommendations for improving patient outcomes
- No treatment plan development or prescription authority required
Schedule/Hours:
- Flexible scheduling: choose shifts between Monday and Sunday
- Self-managed schedule allows you to create your own working hours
- Commit to a minimum number of hours on a weekly or bi-weekly basis
Qualifications:
- Active New York Nurse Practitioner license
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing school
- Minimum of 6 months recent nurse practitioner experience preferred
This per diem position offers a highly competitive compensation of $80$90 per completed evaluation, accompanied by a mileage stipend and compensation for administrative tasks.
