Schedule:

Key Responsibilities:

  • Conduct comprehensive home health risk evaluations for Medicare Advantage members
  • Assess patients prior and current medical history, overall health conditions, and medication usage
  • Deliver patient education focused on medical conditions and medications
  • Identify potential gaps in care, providing constructive recommendations to address them
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in NY
  • Current BLS (CPR) certification
  • Board Certification from AANP or ANCC required
  • Masters Degree in Nursing (MSN) from an accredited institution
  • Minimum of six months current NP experience preferred
