Step into a vital role providing high-quality care and support to Medicare Advantage plan members across Robertson County. This per diem position focuses on conducting home health risk assessments, with flexibility to suit your schedule and professional goals.

Schedule:

• Flexible shifts available between Monday and Sunday
• Create your own schedule
• Options for minimum weekly or bi-weekly hourly commitments

Primary Responsibilities:

• Perform comprehensive annual home health risk assessments in members homes throughout Robertson County, including locations such as Franklin, Calvert, Bremond, and surrounding areas
• Assess prior and current medical history, ongoing conditions, and medication usage
• Educate patients about their health conditions and treatment regimens
• Identify and highlight any gaps in care, making recommendations to address these needs
• Focus exclusively on single-visit assessmentsno treatment plan development or prescriptions required

Qualifications:

• Active Nurse Practitioner License issued in TX
• Current CPR certification (BLS)
• Board Certification from AANP or ANCC
• MSN degree from an accredited nursing program
• Minimum of 6 months recent Nurse Practitioner experience preferred
• Strong interpersonal and assessment skills, with a commitment to patient education
