This per diem position is ideal for an experienced Nurse Practitioner seeking flexibility and the ability to create a personalized work schedule, while serving the vibrant community in and around Glynn County, including Brunswick and the Golden Isles.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose shifts according to your availability
- Minimum weekly or bi-weekly commitment required
Key Responsibilities:
- Perform home health risk assessments for Medicare Advantage plan members
- Complete each assessment visit in approximately 45 minutes
- Assess medical history, chronic conditions, and current medications
- Teach and educate patients about their medical conditions and prescribed treatments
- Identify gaps in care and provide recommendations to optimize patient health
- No need to develop treatment plans or prescribe medications
Qualifications:
- Licensed Nurse Practitioner in Georgia (GA)
- Board certified (AANP or ANCC)
- Minimum of 6 months recent NP experience preferred
- MSN from an accredited nursing program
- Current BLS (CPR) certification
