Bring your clinical expertise and compassionate care to a dynamic per diem opportunity, conducting home health risk assessments for Medicare Advantage plan members. In this role, youll have the unique flexibility to create your own schedule, with shifts available any day of the weekideal for balancing professional and personal commitments.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling options
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive annual home health risk assessments
• Review prior and current medical history, medical conditions, and medications
• Educate patients about their conditions and prescribed medications
• Identify care gaps, offering recommendations for followup and resources
• Focus solely on assessmentno treatment plan development or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in WV
• Current CPR certification (BLS)
• Board certification through AANP or ANCC
• Minimum of six months experience as a nurse practitioner preferred
• MSN degree from an accredited nursing school

Each assessment averages 45 minutes, and assignments are located throughout Monongalia Countyincluding Morgantown, Brookhaven, Cheat Lake, and nearby communities. Youll conduct visits in a variety of home settings, potentially including annual health assessments or post-discharge evaluations.

Enjoy highly competitive compensation$80-$90 per completed evaluation, plus a mileage stipend and administrative time reimbursement.
