Deliver high-impact care in a flexible per diem capacity as an experienced Nurse Practitioner specializing in Home Health risk assessments. This opportunity involves visiting Medicare Advantage members throughout Fayette County, including areas such as Uniontown, Connellsville, and Masontown. The role offers the unique advantage of self-scheduling with flexible shifts available any day of the weekideal for professionals seeking autonomy and work-life balance.
Schedule:
- Choose flexible shifts between Monday and Sunday
- Create your own work schedule
- Minimum commitment options (weekly or bi-weekly hourly commitment)
Key Responsibilities:
- Perform comprehensive annual risk assessments in patients homes, typically lasting around 45 minutes
- Review patients medical history, current conditions, and medication use
- Educate patients on health management and medication adherence
- Identify gaps in care and recommend solutionsno requirement to prescribe medications or develop treatment plans
- Complete documentation and provide administrative follow-up as needed
Qualifications:
- Current Nurse Practitioner license in Pennsylvania
- Active Board Certification (AANP or ANCC)
- Valid CPR certification (BLS)
- Graduation from an accredited MSN program
- At least 6 months of recent NP experience is preferred
