Job Description
Experienced Nurse Practitioners are needed to provide home health risk assessments for Medicare Advantage plan members throughout Hendricks County, INincluding Plainfield, Brownsburg, and Avon. This per diem opportunity enables healthcare professionals to set their own schedule, with flexible shifts available Monday through Sunday and a minimum weekly or bi-weekly hourly commitment.

Key Responsibilities:
- Complete comprehensive annual home health risk assessments for Medicare Advantage members in their homes
- Evaluate current and previous medical history, ongoing medical conditions, and medication usage
- Deliver education to patients regarding medical conditions and proper medication protocols
- Identify gaps in care and offer actionable recommendations to optimize patient health outcomes
- No requirement to develop treatment plans or prescribe medications as part of this role
Desired Qualifications:
- Active and unencumbered Nurse Practitioner license in Indiana (IN)
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- Minimum of six months recent NP experience (preferred)
- Master of Science in Nursing (MSN) from an accredited institution
