Job Description
Deliver high-quality home health care as a Nurse Practitioner specializing in risk assessments for Medicare Advantage plan members in Hendricks County, IN. This per diem opportunity allows you to create your own schedule, with flexible shifts available seven days a week. Work independently in locations including Plainfield, Brownsburg, and Avoneach shift scheduled within these communities based on member needs.
Schedule Details:
- Flexible day shifts offered Monday through Sunday
- Set your own availability
- Choose weekly or bi-weekly minimum hours based on your preference
Role Responsibilities:
- Complete annual health risk assessments in patient homes throughout Hendricks County
- Review members current and previous medical histories, conditions, and medication lists
- Educate patients on their health conditions and medication regimens to promote understanding and compliance
- Identify and document care gaps, offering recommendations as needed
- Visits may include annual assessments or post-discharge follow-ups; no treatment planning or prescribing required
Qualifications:
- Licensed Nurse Practitioner in the state of Indiana
- Board Certification through AANP or ANCC is required
- Current BLS/CPR certification
- MSN degree from an accredited nursing program
- At least six months of recent Nurse Practitioner experience preferred
